Find a Therapist NETWORK you must have
the following:
- A
current and valid license or
certificate to practice in your
state
- Malpractice
insurance coverage
During
the registration process, you will be
asked to provide your billing
information.

available, prior to beginning the
registration process:

carrier, policy number and expiration
date.
-Billing information (Your credit card
number, expiration date, billing
address, etc.)
